The Origins of Nursing in Early Islam: The Legacy of Rufaidah Al-Aslamia

At a time in history when the world had not yet developed organized medical systems, hospitals, or the professional concept of nursing, the foundations of structured patient care were already being laid in the early Islamic community of the 7th century. Long before modern nursing institutions emerged in Europe, a remarkable Muslim woman established principles of patient care, medical organization, community health, and nursing ethics. This pioneering figure was Rufaidah al-Aslamia, widely regarded as the first Muslim nurse and one of the earliest known founders of organized nursing in human history.

Early Life and Medical Background

Rufaidah al-Aslamia was born around 597 CE in the Arabian city of Yathrib, which later became known as Medina after the migration of Muhammad from Mecca in 622 CE. She belonged to the Banu Aslam tribe of the Khazraj clan, one of the major tribes of Medina.

Her father, Saad al-Aslami, was a respected physician and surgeon of the time. Working alongside him, Rufaidah gained practical knowledge of medicine, wound care, and surgical assistance. Through years of assisting her father in treating patients, she developed clinical skills and a compassionate approach toward the sick and injured. These early experiences shaped her understanding that healthcare was not only about treating disease but also about offering dignity, empathy, and psychological support to patients.

Establishment of the First Medical Care Tent

During the early years of the Islamic community, Rufaidah began offering organized healthcare services. She established a medical treatment tent in the courtyard area of Al-Masjid an-Nabawi. This tent functioned as an early medical clinic where patients received treatment for illnesses and injuries.

The facility included basic medical supplies such as bandages, medicinal herbs, ointments, and cotton for wound dressing. Patients were provided shade from the intense desert heat and access to clean water. Rufaidah personally supervised the treatment of patients and ensured that those who were poor or vulnerable received care without cost. Historians often describe this medical tent as one of the earliest forms of a field hospital or mobile healthcare unit in recorded history.

Nursing and Medical Care During Early Islamic Battles

Rufaidah’s role became especially important during the early defensive battles fought by the Muslim community. She organized teams of trained female volunteers who accompanied Muslim forces to provide medical support for wounded soldiers.

These services were particularly evident during major historical battles such as the Battle of Badr, the Battle of Uhud, the Battle of the Trench, and the Battle of Khaybar.

Near the battlefield, Rufaidah and her team would establish tent hospitals where wounded soldiers were treated. Their responsibilities included stopping bleeding, cleaning wounds, applying bandages, monitoring patients, and providing long-term care for those who required recovery time. Severely injured patients were sometimes isolated in separate tents to prevent infection and ensure careful observation.

In many ways, these services resembled what modern medicine now calls emergency response care and battlefield triage.

Recognition and Support from Prophet Muhammad

The medical work carried out by Rufaidah was not only tolerated but actively supported by Muhammad. Historical narrations indicate that the Prophet granted women like Rufaidah the freedom to participate in medical assistance during battles.

He deeply appreciated the humanitarian role played by nurses in caring for wounded soldiers. According to historical accounts, when companions were severely injured in battle, the Prophet sometimes ordered that they be taken directly to Rufaidah’s medical tent for treatment.

One famous example involves Saad ibn Muadh, who suffered a serious arm injury during the Battle of the Trench. The Prophet instructed that he be placed in Rufaidah’s care so that she could treat and monitor his condition.

The Prophet himself would visit injured companions at her medical tent to inquire about their health and offer prayers and encouragement.

Equal Recognition in War Rewards

Rufaidah’s contribution was considered so significant that the Prophet granted her and her team a share from the war booty obtained after battles. Remarkably, historical sources state that their share was equal to that given to soldiers who fought in the front lines.

This recognition demonstrated that medical service and humanitarian work were viewed as essential contributions to society and the defense of the community.

Establishment of the First Nursing Training Program

Beyond her direct medical work, Rufaidah also played a crucial role in educating and training other women. She organized groups of volunteer women and trained them in patient care techniques.

These women were taught how to clean wounds, dress injuries, provide nourishment to patients, maintain hygienic environments, and offer emotional comfort to the sick. Over time, this informal training system developed into what historians consider the earliest organized nursing education program in Islamic history.

Among those who benefited from such training were women from the early Muslim community, including female companions of the Prophet.

Development of Early Nursing Ethics

Another remarkable contribution of Rufaidah was her emphasis on ethical conduct in healthcare. She encouraged nurses to treat patients with compassion, honesty, and respect. She stressed that a nurse’s duty was not limited to physical treatment but also included psychological care and emotional support.

Her teachings emphasized qualities such as sincerity, confidentiality, patience, and empathy—values that today form the foundation of modern nursing ethics.

In many ways, these principles were articulated centuries before the formal development of nursing codes in modern medical institutions.

Community Health and Social Welfare

Rufaidah’s work extended far beyond battlefield medicine. During times of peace, she focused on community healthcare in Medina. She regularly visited different neighborhoods to provide treatment for the sick and assist those who could not afford medical care.

Her services were particularly directed toward vulnerable members of society, including orphans, the poor, and individuals with disabilities. She also educated communities about hygiene and the prevention of disease.

Historians believe that her approach represents one of the earliest examples of organized community healthcare and mobile medical services.

The contributions of Rufaidah al-Aslamia occupy a unique place in the history of medicine. Long before the development of modern hospitals and nursing schools, she established a model for compassionate patient care, organized medical services, nursing education, and ethical medical practice.

For this reason, she is widely regarded in the Islamic world as the pioneer of nursing and one of the earliest figures in global medical history to formalize the principles of nursing practice.
